SUBJECT:
Title
CONSIDER APPROVAL OF AMENDMENT NO. 3 TO AGREEMENT FOR CONTRACT SERVICES BETWEEN THE CITY OF CARSON AND OCCUPATIONAL HEALTH CENTERS (#17-036) (CITY COUNCIL)

Body
I. SUMMARY

The purpose of this item is to approve Amendment No. 3 to a contract with Occupational Health Centers of California ("OHCC") a subsidiary or "managed professional entity" of Concentra, to increase the term and compensation to the agreement.
The contract was originally approved in early 2017 with U.S. HealthWorks (Exhibit No. 1). In early 2018, U.S. HealthWorks was acquired by a company called Concentra and then assigned to OHCC, a subsidiary or "managed professional entity" of Concentra. The City approved the assignment in Amendment No. 1 (Exhibit No. 2). In February 2020, City Council approved Amendment No. 2 extending the contract by one year to February 27, 2021 and increasing the contract sum to cover an additional year of costs (Exhibit No. 3).
The proposed Amendment No. 3 will extend the contract term by one additional year to February 27, 2022 and increase the contract sum to cover the additional year of costs (Exhibit No. 4).
