CONSIDER APPROVING AMENDMENT NO. 1 TO THE SUBRECIPIENT AGREEMENT BETWEEN THE CITY OF CARSON AND VENICE FAMILY CLINIC

I. SUMMARY
In adopting the Community Development Block Grant (CDBG) 2021-2022 Annual Action Plan at its April 20, 2021 meeting, City Council approved entering into agreements with several non-profit service providers ("subrecipients") to support their operations with CDBG funds. One of these providers, South Bay Family Health Care (SBFHC), has merged with and became a part of Venice Family Clinic. This requires amending the subrecipient agreement with SBFHC so that it is with Venice Family Clinic. Services provided are not impacted by this change.
II. RECOMMENDATION
Recommendation
1. APPROVE Amendment No. 1 to the Subrecipient Agreement between the City of Carson and Venice Family Clinic; AND
2. AUTHORIZE the Mayor to execute the Subrecipient Agreement between the City of Carson and Venice Family Clinic.

IV. BACKGROUND
At its meeting on April 20, 2021, City Council approved the 2021-2022 Community Development Block Grant Annual Action Plan, which included allocations of CDBG funding to several non-profit service providers. Among these was an allocation of $15,000 to South Bay Family Health Care to support the operations of the Carson Wellness Center, located on the campus of Carson High School. The City entered into a subrecipient agreement with SBFHC spelling out the terms and conditions of this funding (Exhibit No. 1).
On November 2, 2021, staff received a letter from SBFHC indicating that as of November 1, 2021, it would become a part of Venice Family Clinic (Exhibit No. 2). This action necessitates amending the existing subrecipient agreement with SBFHC, thus Amendment No. 1 (Exhibit No. 3) has been drafted for that purpose.
